Join Dr. Carrie L. Cormack for From Clinic to Comfort: Integrating Palliative Care Across the Primary Care Journey, an engaging session that explores how primary care providers can identify patients who would benefit from palliative care and connect them with the right support at the right time. Drawing on her national leadership in palliative care education and the End-of-Life Nursing Education Consortium (ELNEC), Dr. Cormack will share practical strategies for symptom management, goals-of-care conversations, and caregiver support. Attendees will leave with actionable tools to integrate palliative care principles into everyday practice and improve outcomes for patients and families.

 Learning Objectives:

At the conclusion of this session, participants will be able to:

  1. Identify at least five practical referral triggers for palliative care in primary care settings.

  2. Describe the core palliative care services (symptom management, goals-of-care communication, caregiver support, care coordination) and how they impact patient, caregiver, and system outcomes.

  3. Explain what ELNEC is, summarize its curriculum structure, and identify two ways to integrate ELNEC content into NP education and onboarding.

  4. Apply a simple referral workflow and scripting  for introducing palliative care to patients and families in a primary care visit.
